Submit a Claim Form	If you meet the eligibility requirements described below in the Notice, the *only* way for you to receive compensation is to submit a Claim Form.	**Postmarked by May 7, 2007**
Object	File an objection with the Court explaining why you do not like the settlement.	**Postmarked by June 5, 2006**
Exclude Yourself	If you do not want to participate in the settlement, you can exclude yourself from the settlement. If you do this, you will not receive any compensation, but you will keep your individual right to sue the defendants regarding the claims in the lawsuit.	**Postmarked by June 5, 2006**
Appear in the Lawsuit	You can appear in this lawsuit on your own or through your own lawyer. You also can ask to speak in Court about the settlement.	**Postmarked by June 5, 2006**
Do Nothing	You will receive no compensation and you give up the right to sue the defendants on your own regarding these claims later.
